Post acute services
Northwell Health offers a range of after-hospital services to meet your individual needs. One or more of the following services may be the right choice for you. Speak to your physician or discharge planner about these and other options available to you.
Acute inpatient rehabilitation
Provided at Glen Cove, Phelps and Staten Island hospitals, acute rehabilitation is designed for individuals who need frequent physician monitoring, 24-hour nursing and intensive physical, occupational and/or speech therapy to maximize recovery. Subacute inpatient rehabilitation
Subacute rehabilitation is for patients who need a less intense, less medically supervised rehabilitation environment. Services are provided at the Stern Family Center for Rehabilitation on the North Shore University Hospital campus in Manhasset, and the Orzac Center for Rehabilitation on the Long Island Jewish Valley Stream campus.
Services include up to seven-day-a-week physical/occupational therapy and speech pathology as clinically indicated, restorative nursing, wound care, social work and nutritional counseling. The subacute length of stay lasts until the patient is ready to return home or to a long-term setting. Subacute rehabilitation often is followed by home care or outpatient rehabilitation. Outpatient rehabilitation
Comprehensive outpatient rehabilitation services are offered at 21 easy-to-access locations throughout Long Island, Queens, Staten Island, Manhattan and Westchester. Our fully equipped facilities are staffed by licensed therapists experienced in the latest rehabilitation techniques. Our services include joint replacement therapy, neurological and stroke rehabilitation, sports therapy, spine care, pain management, brain injury rehabilitation, lymphedema therapy, oncology rehabilitation, women’s health, and vestibular and balance rehabilitation. Home care
Northwell Health At Home provides a scope of home care services for all patients, from infants to the elderly. Services may include nursing, physical and occupational therapy, speech/language pathology, wound and skin care, infusion services, social work and home health aides. Services may be covered through Medicare, Medicaid and Managed Care insurance plans. Private pay home aide services also are available.

Hospice
Northwell Health Hospice Care Network offers patients with advanced illness in Nassau, Suffolk and Queens expert, compassionate care to alleviate symptoms and improve quality of life. A team of nurses, social workers, home health aides, chaplains, dietitians and volunteers is available to assist the patient and family.

Labs
Northwell Health Labs is the leading health system laboratory provider in the New York area, offering a comprehensive testing menu. We provide laboratory testing for hospitals, nursing homes, physicians and ambulatory locations, which means your health care provider has seamless access to your test results.

The Center for Emergency Medical Services (CEMS) provides a range of ambulance services to all health care facilities with highly trained emergency medical technicians and paramedics.
